Who is Hackney Podiatry? 

 

Hackney Podiatry (registered with Companies House as SC Chiropody Ltd) is a healthcare company providing podiatry services on a fee paying basis to the public. The director and lead Podiatrist is Sarah Crookes; a HCPC registered podiatrist (reg number CH15951). 

 

What do we do with your information?

 

Hackney Podiatry processes personal information so that health services can be provided to patients.  With your consent, Hackney Podiatry will use your personal information to contact you about your appointments, contact your GP, make onward referrals and keep you up to date with changes to the practice. 

 

What information does Hackney Podiatry collect from you?

•    lifestyle and social circumstances

•    goods and services purchased from Hackney Podiatry

•    financial transaction details

 

We also process sensitive information that may include:

•    physical or mental health details

Why do we ask you for this information?

 

The information we collect is necessary in order to: 

 

  • Identify you 
  • Contact you or a next of kin in an emergency 
  • Provide safe and effective treatment relative to existing medical conditions and prescribed medication

 

As a healthcare practice, Hackney Podiatry is required by law to keep records of your treatments and this information forms part of this. 

 

Who else sees my information? 

 

Sometimes it is necessary to share some of your information in the case of onward referral. This may be a GP, physiotherapist or other therapist deemed appropriate. 

If you are purchasing bespoke orthotics, your personal information and details of your presenting foot complaint are shared with chosen Orthotics Laboratories. This information is stored by them for the purposes of orthotic manufacture and patient identification. 

If you are under the age of 18, attending without a guardian, information may be shared with an individual who has parental control if necessary. 

When it is necessary to share your data, it is in compliance with all aspects of the Data Protection Act (DPA). Hackney Podiatry is also registered with the Information Commissioner's Office (ICO).

 

Is my information shared without consent or sold to third party companies? 

 

Your information is never shared without consent and is never sold to any third party companies. Your data will never be sold by Hackney Podiatry for marketing purposes.

 

How is my data stored? 

 

Hackney Podiatry uses a practice management software called Cliniko, which is password protected. All records are electronic. Any paper records are digitised and stored in the electronic format. Some data is stored outside of the European Economic Area (EEA) and these servers are all compliant with the relevant data protection laws.

 

Who is the data controller?

 

Sarah Crookes, the lead podiatrist is responsible for overseeing our data protection. She can be contacted at the clinic or by email at sarah@hackneypodiatry.co.uk

 

I want to remove my information from your company, how can I do that?

 

Hackney Podiatry is legally required to keep your medical records for a period of 7 years. If you wish to have your health care records, you can submit a request in writing and Hackney Podiatry has up to 28 days in which to respond to your Subject Access Request (SAR). Does your website store information on my computer?

 

Hackney Podiatry’s website does have cookies which will stay on your computer and the Hackney Podiatry IP address may be stored in your system too (depending on your settings).
